This is Windows NT 4.0 Workstation with Service Pack 6a (SP6a). This is one rocksolid operating system this is the first the OS to have the Windows 95 GUI (interface). This is one stable OS out there and it's one my favorites. Windows NT 4.0 was released around July 1996.
